2026 General Waiver (Final Version for Digital Signing)
G&J Studios reffer to as Studio
Accident Waiver, Release of Liability, and Indemnity Agreement
I understand that participating in dance classes, private lessons, camps, workshops, events, or any other activities at G&J Studios involves inherent risks, including but not limited to falls, collisions, overexertion, equipment issues, or negligence of others. These risks may result in personal injury, illness (including pandemics), disability, death, or property damage.
Assumption of Risk
I HEREBY ASSUME ALL RISKS associated with my (or my minor child’s) participation in any activity at the Studio.
Health Certification
I certify that I (and/or my minor child) am/is physically fit, have/has sufficiently prepared for the activity, and have/has no health-related reasons or problems that preclude participation. I further certify that I (and/or my minor child) am/is in good health and do/does not pose a health risk to others.
Release and Waiver of Liability
In consideration of being permitted to participate, I, on behalf of myself, my executors, administrators, heirs, next of kin, successors, and assigns (and, if applicable, on behalf of my minor child), hereby waive, release, and discharge G&J Studios, its owners, instructors, employees, volunteers, agents, sponsors, and event holders from any and all liability for any injury, sickness, death, disability, personal injury, property damage, or loss arising out of or connected with participation in any activity at the Studio, including claims based on negligence, to the fullest extent permitted by law.
Indemnification
I agree to indemnify, hold harmless, and promise not to sue the released parties from any and all claims, liabilities, or damages resulting from participation, whether caused by negligence or otherwise.
Additional Responsibilities for Independent Instructors, Group Leaders, and Event Organizers
(Only applicable if you are signing as an instructor, group leader, or event organizer)
I agree to the following:
* I am fully responsible for the actions, behavior, supervision, and safety of all participants I bring, as well as the quality and safety of any instruction or program I provide.
* I will ensure every participant under my direction reads and signs the Studio’s General Waiver. I will maintain records of all signed waivers and provide copies or a participant list to G&J Studios upon request.
* The Studio is not liable for any claims involving me, my clients, or my organized activities.
* I will indemnify, defend, and hold harmless G&J Studios from any claims, damages, losses, or expenses (including attorney fees) arising from my use of the premises, instruction, or participant activities.
* I am acting as an independent contractor; the Studio provides only rental space.
* I maintain adequate commercial general liability insurance (minimum recommended: $1,000,000 per occurrence) and will name G&J Studios as an additional insured upon request.
* I will comply with all Studio rules and policies. I am responsible for any damage to Studio property caused by me or my participants.
Acknowledgment
I certify that I have read this entire document, fully understand its content, and am aware that this is a release of liability and a legally binding contract. I sign it of my own free will.
Signing Section
If signing for a Minor (under 18):
I confirm I am the parent or legal guardian of the minor named above and have authority to sign on their behalf.
If signing as Independent Instructor / Group Leader / Event Organizer:
I confirm I am signing both in my individual capacity and on behalf of all participants in my group/event. I have the authority to bind them to these terms.
